The information you supply will be used to register you against the training and/ or examination you desire. Applicants are under no obligation to apply for both training and examination, taking either as required.

4 Identity Checks>
In order to prevent learner malpractice - specifically:
'Pretending to be someone else or arranging for another to take their place in an examination’>
Identity checks are made as a normal part of the process of qualifications through Stroma Certification.
Where there are formal invigilated examinations, identity checks are made as part of the examination process either by Stroma Certification or exam centres.
In order to facilitate this, as well as a better learning experience, applicants should bring photo ID to any training/ examinations attended and provide a passport style photograph or scan of photo ID below:

5 Training Course>
Applicants wishing to undertake Stroma Certifications UK Government approved CSH training, should mark the appropriate option bellow. Applicants should also indicate the preferred date and location for the event.

5.1 Pre-Qualification Competence Requirements
No pre-qualification criteria are required to undertake any of the listed training courses.

6 Examination
Applicants wishing to undertake Stroma Certifications UKAS approved ISO 17024:2012 CSH examination, should mark the appropriate option bellow. Applicants should also indicate the preferred date and location for the event.

6.1 Pre-Qualification Competence Requirements
Applicants wishing to undertake their examination(s) with Stroma Certification who do not plan to, or have not undertaken the appropriate training with Stroma Certification in the past must confirm one off the following two options:

6.2 Examination Options
Please confirm if your examination is a new examination with Stroma Certification, or you are returning for a resit.

If you chose option B, you will be contacted to discuss suitable options. Where possible we will offer an examination date following a training course, however where this is not suitable, bespoke options are available.

6.3 Examination Requirements
Candidates are required to provide photographic ID to confirm identity. This must be either a passport or photo ID driving license.
A candidate may only bring the following items to the examination:
• A hard copy of the current Technical Guidance (Including the Addenda)
• The Water Efficiency Calculator for new dwellings (or AD Part G),
• A pen.
• A calculator.
Where applicable all other items are restricted and must be left with the invigilator for collection at the end of the examination.

7 Granting, Maintaining, Renewing, & Suspending/ Withdrawing Certification
An applicant for the ‘Code for Sustainable Homes’ examination should be aware and agree to the following items:
• Details within Section 4, ‘SHM4 - Scheme Handbook - CSH’. This information can found in the CSH ‘ Members Pack’.
• Comply with the provisions of the certification scheme as detailed in ‘SMH4 - CSH Scheme Handbook’. This information can found in the CSH ‘ Members Pack’.
• The candidate on completing the examination, and achieving a pass will not make claims regarding certification outside their scope off certification as detailed on the ISO 17024:2012 Certificate of Certification for CSH.
• The candidate on achieving an ISO 17024:2012 Certificate of Certification for CSH, will not use this certification in a manner that will bring Stroma Certification into disrepute, or make any statements regarding certification that Stroma Certification considers misleading or unauthorised.
• The candidate aggress to discontinue the use of all claims to certification that contains Stroma Certifications reference in the event that the certificate is suspended or downdrawn, and agrees to return the ISO 17024:2012 Certificate of Certification for CSH upon request, or suitable proof as agreed with Stroma Certification) it has been destroyed.
• The candidate on achieving an ISO 17024:2012 Certificate of Certification for CSH, will not user the certificate in a misleading manner.

8 Code of Conduct
Applicants applying for training and/ or the examination course should read and be aware of the ‘Code of Conduct’. This information is can be read in Section 8, ‘SHM4 - Scheme Handbook - CSH’ located within the CSH ‘ Members Pack’.

9 Equality, Diversity & Fairness
Stroma Certification operates an ‘EDF1 - Equality, Diversity and Fairness Policy’ to ensure the equality of all applicants who attended training or examination events. This document can be found in the CSH ‘ Members Pack’.
Should you have any specific requirements please provide these in the table below so that we can accommodate these prior to your attendance of a training course and/ or examination;

10 Training & Examination Fees
The course and examination fees are as follows. All fees exclude VAT. Stroma Certification will issue a VAT invoice for every confirmed application.
• CSH Assessor Training Course Fee: £995 (3 days)
• CSH Designer Awareness Training Course Fee: £695 (2 days)
• Exam Only Fee: £300 (1 day)
Examination: Should you fail the examination; your first re-sit will be free of charge; with each subsequent examination fee charged at £195 assuming the examination is held on a scheduled examination date. Bespoke examinations will be charged at the standard fee (£300).
An applicant, who wishes to apply to Stroma Certification for certification as a CSH Certified Assessor, should read and be aware of the appropriate fees necessary for membership. This information can found in the Section 5 ‘SHM4 - Scheme Handbook - CSH’ located in the CSH ‘ Members Pack’.

Payment Options *

12 Booking Confirmation
We will reserve your place on the appropriate course upon receipt of the form and confirm your booking within 14 days. If you have not received confirmation two weeks after booking, please telephone Stroma Certification Ltd for confirmation of receipt.

13 Changes to the Listed Programme
Stroma Certification Ltd reserves the right to make changes to any course dates as necessary.

14 Cancellation
A fee of 50% of the full applied for training and/ or examination fee will be charged for any cancellation. The full fee will apply to cancellations of less than 5 working days, failure to attend without prior notification, or leaving after attending part of your training and/ or examination. If you must cancel your booking, please notify Stroma Certification in writing. These conditions will not apply if the training and/ or examination event is postponed or cancelled by Stroma Certification. However, you may submit in writing at any time before the event, the name of a colleague to take your place.

16 Acceptance of Requirements
By signing this document, I confirm I have read and I agree with the requirements therein. I enclose a cheque made payable to Stroma Certification to be banked if I am successful with my application. Code for Sustainable Homes, examination only: I confirm that I am aware of the conditions for granting, maintaining, renewing and suspending/ withdrawing certification.
